Headache action plan
This action plan is for children with frequent headaches or a diagnosis of migraine headaches. This plan should be filled out and signed by the child's doctor.

This plan is for children with a history of seizures. This plan must be filled out and signed by the child's doctor.

Action plan for children with allergies that can result in severe allergic reactions. This plan must be filled out and signed by the child's doctor.

Action plan for children with asthma that need an inhaler at school. This plan must be filled out and signed by the child's doctor.

Form needed for administration of any medication to be given during school hours.

Welcome to CES! My name is Marcie Garrett and I am your school nurse. I have been at CES for 4 years, and I have been an RN for 20+ years. I have a BSN in nursing and I am currently working on my master's degree. I love working with children and I am passionate about school nursing. I am here to help your child be present, healthy, and safe. I am always here to help in any way I can. Please feel free to reach out with any needs you may have this year. One note for all parents about medications: All medications, prescription and over the counter, MUST be brought to the clinic by a parent/guardian/adult. Students are not allowed to carry medications in their bookbags without prior approval from their doctor and without having a plan on file in the clinic. If your child has allergies, chronic conditions, or daily medications to take while at school, I will need to have paperwork and/or action plans from their doctor on file. Please contact me with any questions or concerns about this policy.
