K - 12 In-District Enrollment

• Two forms of Proof of Residency are required. 
• One or both parents/guardians must be listed on Proof #1 and Proof #2.
• If you are living with someone, you must submit an affidavit of residency plus two (2) printed forms of Proof of Residency in that person’s name.

Proof #1 Examples 

mortgage statement, deed, lease, rental agreement, property tax statement

Proof #2 Examples

City of Cartersville utility bill, power bill, water bill, gas bill

• Birth certificate or passport for each child
• Social Security Card for each child or signed waiver
• State issued ID, driver's license or passport for enrolling parents/guardians
• Guardianship paperwork, if applicable
• IEP and/or 504 paperwork, if applicable
• For high school students: Current transcript or most recent report card

Students coming from outside Georgia or from another country must complete additional Georgia Department of Health forms.
